For over 43 years, we have cared for hospice patients across the Portland Metropolitan area, bringing interdisciplinary and holistic hospice services to patients and their support systems.
Care Partners is seeking a full-time Registered Nurse for the position of RN Case Manager to provide coverage in the Portland metro area. This position has candidates in the field providing direct patient care.

We want to collaborate with you to find a full-time schedule that meets your needs. The onboarding and training process will involve hands-on learning with an RN preceptor, directly supported by our Clinical Managers and will be tailored to your level of experience until you feel comfortable working independently

*Preferred applicant will be required to pass a background check and pre-employment drug screen, following an offer of employment.
**New employees are required to complete tuberculosis screening and provide proof of Hepatitis B vaccination or signed declination.
